Pentel Energel Alloy Gel Pen - 0.7mm 

Considering this being my everyday carry when I start hospital clinicals and fire department vehiculars. It's metal, durable, and writes so smooth. Pilot G-2 gel pens seem to be the gold standard for this field, so I'll carry both just in case.
